I used a very similar item and both the negative and positive reviews have truth to them. Here's my 2 cents worth.
Drill a tiny pilot hole. I think you could hammer in a small nail for a starter hole. Even with a hole drilled it took some effort for me to work the wire through.
If you use a twisting motion to work the wire in, it's important to keep the wire straight (horizontal) or else it WILL make an enlarged hole from spinning around at an angle. Even with care, the hole is definitely bigger than the pinhole I was imagining, but way smaller than a hole from an anchor.
I needed to grip the wire with pliers for the final push in. It's important to get that bend in the wire right up against the wall and I couldn't do that with my bare hands.
You need empty space behind the drywall but this thing does not have to spin a full 360.º You need empty space behind and above the point of insertion. The wire will slide in along its curve but it takes a little care and effort. Once you do it you'll get a feel for it. You could even practice on an old piece of drywall or heavy cardboard.
I used these on standard drywall, not sure if it's half inch or five eighths.
These work in the particular situations where they work. If there's nothing behind your drywall, a nail is not going to work unless you're hanging a piece of paper. And if there's a stud behind your drywall, go with that--a stud trumps all.
I used these to hang two shower caddies at the back of the tub, just above the tub surround. They are holding up great with lots of bottles of shampoo, etc. weighing them down and being lifted in and out. I was not sure if they are designed to hold something that gets used, as opposed to a picture just quietly hanging there.

I picked up a dozen in Home Depot for less than $5. Same concept, just a lot cheaper!
Two of them are holding a huge heavy shelf in my kitchen. The hole they make depends upon the weight of what they're holding. I'm sure it's pinhole sized if you're hanging something that weighs a few ounces (in which case, you don't need this - use a pushpin!)
The holes from the shelf are bigger than pin-sized, but it IS a heavy shelf. The hole is still WAY smaller than the hole I'd make using any kind of a hook that would hold such a heavy shelf.
It also takes a little practice to figure out where to hang the hooks (the drop of the hook is below where you might want the thing to actually hang.) This is a small detail, but if you need two of these things and you don't do it right, your object is gonna be crooked. But on the bright side, you won't have a huge hole to patch when you redo it!
Recommended product (but look around for the knockoffs.)

After reading some bad reviews from a few people I was quite scepticle about these items but decided to give them a go.
I cant speak for plaster, but as far as I know these are meant for dry wall anyways.
I have hung a 30 pound item on one hook which is extremly secure and was so easy to use that I have now vowed never to purchase any of the old fashioned hooks again.
I got a total of 40 hooks (2X20 pack) and have hung 4 items so far.
The hooks are very easy to get through the dry wall and just as easy to remove when needed.
The hole that is left is only that of a small nail and can easily be touched up while retaining your hooks in the same condition as when you put them in.
Over all I give this a 5/5 and would suggest this item to other buyers, I was also shocked at just how strong the metal is, I attemped to cut through one with a wire cutter, but the blades of the cutter just bent around the metal of the hercules hook.

I got a set as a gift and used them out of curiosity (I never buy stuff from infomercials). They worked MUCH better than standard dry wall nails and/or dry wall cleats with screws! The hole is SUPER tiny in comparison and if you change your mind on the location of the hanging object, simply twist, pull out the hook and insert to the new location, the hole is so small no patching needed!
I EASILY put these through 3/4" drywall, I'm not sure why people are having issues installing these, it doesn't get any easier.
I WILL NOT test the 150lb load capacity statement, for heavens sake, if your hanging something THAT heavy on dry wall, use a stud finder and anchor it properly into a stud.
I would also echo to keep a set of small nails and a hammer handy, just in case you hit a stud with these hooks, no problem, just sink a small nail in the stud...voila.

These things are terrible. I had one go in easily and work fine, but the other 3 I've tried I still can't get to work. The big problem is these hooks need about a 6" diameter circle behind the wall from wherever you screw them, as they need to twist. They also, of course, don't go through studs.
Now I have 3 holes in my wall, one of them MUCH larger than a screw hole, and still no hook. I also have several cuts on my hand from trying to push these damn things in. Think of bending sharp clothes hangers, which is exactly all these are.
With screws I would have been done in 2 minutes, now I've spent about 20 and have one hook in. Throwing the rest out, going to buy some $0.50 screws instead. Horrible product in every way, expect under rare conditions where they are in the perfect spot to work (which is rarely ever centered).
